چکیده

Cardiac and aortic pressures were recorded after stellate gangliooectomy and vagotomy. Acute heart block was produced by injecting the atrioventricular node, and atrial and ventricular systoles were controlled electronically to occur independently or in any desired relationship. Angiocardiograms recorded on video tape after injections of 4 ml 69% Renovist into the left ventricle were analyzed with a videodensitometer able to detect small refluxes of contrast medium into the left atrium and correlate them with phases of the cardiac cycle. When ventricular driving was temporarily suspended but atrial driving continued, pressure records indicated mitral valve closure after each atrial systole, but reflux of contrast medium into the atrium occurred after each systole not followed by a normally sequenced ventricular systole. Driving with a 2 : 1 atrioventricular stimulation resulted in reflux, with the alternate atrial contraction dissociated from ventricular systole. Thus, the mitral valve was not effectively closed by atrial systoles that were not followed by normally sequenced ventricular systoles. ADDITIONAL

بررسی عوامل موثر در افزایش سطح دریچه و پیدایش نارسایی میترال بعد از بالون میترال والولوپلاستی در 110 بیمار در مجتمع بیمارستانی امام خمینی

Background: Percutaneous Transeptal Mitral Commisurotomy (PTMC) has been used in treating mitral stenosis with variable success, its main complication being mitral incompetence. There is a need to define the subgroup of the patients who benefit mostly from the procedure. Methods: We studied 110 patients (age 17 to 60 years mean 33.2) with mitral stenosis. PTMC was performed though femoral vein....

Comparison of left atrial pressure and wedge pulmonary capillary pressure. Pressure gradients between left atrium and left ventricle.

Simultaneous records of wedged pulmonary capillary pressure and left atrial pressure have been made in I2 patients. The pressure changes in the left atrium preceded those in the pulmonary capillary bed by o-o6 to O0I4 sec. This delay, together with the damping of the wedged pulmonary capillary pressure, may result in a false appearance of an end-diastolic gradient between the left atrium and th...

Sound pressure correlates of the Austin Flint murmur. An intracardiac sound study.

Mitral valve motion and pressure correlates of the Austin Flint murmur (AFM) were investigated in nine patients with aortic regurgitation using high fidelity catheter tip micromanometers and the mitral valve echocardiogram (MVE). External phonocardiography demonstrated a mid-diastolic murmur (MDM) in eight subjects and a presystolic murmur (PSM) in five. Maximum intensity of both AFM components...

Electrocardiographic differentiation of the causes of left ventricular diastolic overload.

Left ventricular hypertrophy due to diastolic overload is characterized by the following in lead V6: (a) tall R wave; (b) prominent initial Q wave; (c) minimally elevated concave-upward ST segment; and (d) relatively tall symmetrical T wave. Reciprocal deep S waves are seen in lead V1. This study reflects a further evaluation of these parameters in the four main causes of left ventricular diast...

Diastolic mitral regurgitation. Haemodynamic and angiographic correlation.

A better understanding of the factors involved in ensuring competence of the mitral valve based on knowledge of the function of the leaflets, chordae tendineae, and papillary muscles has expanded the clinical range of systolic mitral regurgitation. For all this, diastolic mitral regurgitation remains a curiosity. Late diastolic mitral regurgitation has been described together with reversal of t...
